The Philippine Deposit Insurance Corporation (PDIC) announced that it will start servicing deposit insurance claims of depositors of three closed banks, namely: Rural Bank of Tagudin (Ilocos Sur) Inc., Rural Bank of Nabas (Aklan) Inc., and Mambajao Community Rural Bank Inc. on September 7, 2011.

PDIC advised depositors of the said closed banks to file their claims at the premises of the banks. The schedules of the claims settlement operations (CSO) are: 1) Mambajao Community RB (Sept. 7 to 13, 2011); 2) RB Nabas (Sept. 7 to 19, 2011); 3) RB Tagudin (Sept. 7 to 20, 2011). Depositors may file their claims during office hours, 8:00 AM to 5:00 PM, from Monday to Friday. After the last date of the CSO for these respective banks, depositors may file the claims at either the PDIC Office located at the 4th Floor SSS Bldg., Ayala Avenue corner V.A. Rufino St., Makati City, during office hours, or via mail.

When filing claims, depositors must submit the following documents: 1) duly accomplished Claim Form; 2) Claims Status Sheet (CSS); 3) original evidence of deposit such as passbook or certificate of time deposit; and, 4) enlarged and clear photocopies of two (2) valid photo-bearing IDs with signature of depositor/claimant. Acceptable IDs are passport, SSS/GSIS ID, and Voter’s ID, among others.

Depositors who are below 18 years old are required to submit either a photocopy of their Birth Certificate issued by the National Statistics Office (NSO) or a duly certified copy issued by the Local Civil Registrar. A parent of a depositor who is a minor is required to sign the Claim Form and the CSS.

PDIC said that additional documents may be required in the course of claims processing. Claim Forms and CSS were earlier distributed to depositors but may also be downloaded from the PDIC website, www.pdic.gov.ph. Information on the CSO is also available at the PDIC website.

PDIC reiterated that all valid deposit insurance claims will be paid.
