Banco Filipino Savings and Mortgage Bank
Claims Settlement Operations

The Philippine Deposit Insurance Corporation (PDIC) will start Claims Settlement Operations for insured deposits with the closed Banco Filipino Savings and Mortgage Bank (BFSMB) on June 15, 2011 (“Claims Settlement Period”).
The Priority Number indicated in the Claim Status Sheet issued by PDIC at the time a depositor files his/her claim/s corresponds to an individually assigned appointment date posted at the bank premises and PDIC website.

Depositors are advised to proceed to the bank premises or designated payout site on their appointment date during the Claims Settlement Period from Monday to Friday, 8:00 AM to 5:00 PM:

Depositors who fail to appear on their appointment date may not be accommodated by the PDIC representatives during the Claims Settlement Period.
The following depositors may proceed to PDIC at the 4th Floor SSS Bldg., 6782 Ayala Ave. corner V.A. Rufino St., Makati City from Monday to Friday, 8:00 A.M. to 5:00 P.M. starting August 29, 2011 until March 18, 2013:
1. depositors who failed to appear on the appointment date during the Claims Settlement Period;
2. depositors whose Priority Number is beyond the Claims Settlement Period; and
3. depositors who failed to file their claims during the Claims Settlement Period.
During the Claims Settlement Period, the following minimum documents along with the Claim Status Sheet should be presented/submitted to the PDIC representatives, to wit:
1. Original copy of the two (2) valid IDs submitted during the filing of the claim;
2. For minor depositors who are below 18 years old, original and clear photocopy of Birth Certificate from the National Statistics Office (NSO) or duly certified copy from the Local Civil Registrar;
3. Notarized/authenticated copy of Special Power of Attorney of depositor or parent of minor, if claimant is not the signatory in the bank records.
Other documents may be required by the PDIC representatives in the course of claims processing.
There are no fees or charges for the processing or payment of all deposit insurance claims. PDIC Claim Forms, pro-forma affidavits and documents are given free of charge.
The public is advised to transact only with authorized PDIC representatives.
In accordance with the provisions of R.A. 3591, as amended, the last day for filing claims for insured deposit in the closed Banco Filipino Savings and Mortgage Bank is on March 18, 2013. After the said date, PDIC, as insurer, shall no longer accept any claim for insured deposits maintained with the said closed bank.
