Notice to the Depositors of the Closed Rural Bank of Mapandan (Pangasinan), Inc. (RBMI)
and Rural Bank of Norzagaray (Bulacan), Inc. (RBNI)


  1. Pursuant to Monetary Board Resolution No. 105 and 104 dated January 27, 2011, ordering the closures of the Rural Bank of Mapandan (Pangasinan), Inc. (RBMI), and Rural Bank of Norzagaray (Bulacan), Inc. (RBNI), the Philippine Deposit Insurance Corporation (PDIC) will start servicing of claims for insured deposits in the closed RBMI and RBNI on March 16, 2011.

  2. Depositors are requested to proceed directly to the premises of the closed banks where they can file their claims with the PDIC representatives during office hours, 8:00 A.M. to 5:00 P.M., during the following schedules:

    Rural Bank of Mapandan, Inc. Pandan Ave., Poblacion Mapandan, Pangasinan March 16 - 18, 2011
    Rural Bank of Norzagaray, Inc. #5 A. Payumo Street, Norzagaray, Bulacan March 16 - 24, 2011

    After the said date, all depositors can personally file their claims at PDIC, 4th Floor, SSS Bldg. Ayala Avenue corner V.A. Rufino St., Makati City from Monday to Friday, 8:00 A.M. to 5:00 P.M. or by mail.

  3. Depositors are advised to present the following minimum requirements to the PDIC representatives when filing their claims:

    1. DULY ACCOMPLISHED CLAIM FORM and CLAIM STATUS SHEET;

    2. ORIGINAL EVIDENCE OF DEPOSIT such as savings passbook, certificate of time deposit;

      For joint accounts: “OR, AND/OR, AND”, passbook/certificate of time deposit should be photocopied as many copies as there are depositors to the account.

    3. ORIGINAL AND PHOTOCOPY OF TWO (2) VALID PHOTO-BEARING IDENTIFICATION DOCUMENTS (IDs) with signature of depositor (e.g. Driver’s License, SSS/ GSIS ID, Senior Citizen’s ID, Passport, PRC ID, OWWA/ OFW ID, Seaman’s ID, Alien Certification of Registration ID, Voter’s ID);

    4. ORIGINAL AND PHOTOCOPY OF THE BIRTH CERTIFICATE FROM THE NATIONAL STATISTICS OFFICE (NSO) OR DULY CERTIFIED COPY FROM THE LOCAL CIVIL REGISTRAR, for depositors below eighteen (18) years old.

    PDIC will not accept claims which are incomplete. Likewise, other documents may be required by PDIC in the course of processing of claims.

  4. In accordance with the provisions of R.A. 3591, as amended, the last day of filing claims for insured deposit in the closed Rural Bank of Mapandan (Pang.), Inc. and Rural Bank of Norzagaray, (Bulacan), Inc. is on January 28, 2013. After January 28, 2013, PDIC, as insurer, shall no longer accept any claim for insured deposits maintained with the said closed banks.
