Notice to the Depositors of the Closed
Rural Bank of Ayungon (Negros Oriental), Inc. (RB Ayungon),
Rural Bank of Lagonoy (Camarines Sur), Inc. (RB Lagonoy) and
Rural Bank of Hinigaran (Negros Occidental), Inc. (RB Hinigaran)
| The Philippine Deposit Insurance
Corporation (PDIC) will start receiving claims for insured
deposits of the closed Rural Bank of Ayungon (Negros Oriental),
Inc. (RB Ayungon), Rural Bank of Lagonoy (Camarines Sur), Inc.
(RB Lagonoy) and Rural Bank of Hinigaran (Negros Occidental),
Inc. (RB Hinigaran).
In obtaining a claim form, depositors must
present the original evidence of deposit (such as savings
passbook or certificate of time deposit) to the PDIC
representative/s. The number indicated on the Claim Form
corresponds to the Filing Date posted at the receiving
sites.
Depositors are requested to proceed
directly to the following sites on their scheduled filing date
where they can file their claims with the PDIC representative/s
during office hours, Monday to Friday:

After the said dates, depositors can file
their claims personally at the Ground Floor, PDIC Bldg., 2228
Chino Roces Ave., Makati City from Monday to Friday, during
office hours.
Depositors should PERSONALLY FILE
their claims, except when depositor is:
below seven (7) years old,
thus, parent should file the claim;
abroad, thus a special power of attorney
of depositor, authenticated before the Philippine Consulate
should be submitted;
incapable of personally appearing before
the PDIC representative/s due to serious physical injury,
condition, ailment, or infirmity due to advanced age. Thus, a
notarized medical certificate stating the health condition of the
depositor and notarized special power of attorney should be
submitted.
Note: Depositors between 7 to 17 years old
must be accompanied by a parent in the filing of the claim.
In filing a claim for insured deposits,
the following minimum requirements should be submitted:
DULY ACCOMPLISHED CLAIM
FORM and CLAIM STATUS SHEET;
ORIGINAL EVIDENCE OF DEPOSIT such
as savings passbook or certificate of time deposit;
For joint accounts: “OR, AND/OR, AND”,
passbook or certificate of time deposit should be photocopied as
many copies as there are depositors to the account.
ORIGINAL AND PHOTOCOPY OF TWO (2) VALID
PHOTO-BEARING IDENTIFICATION DOCUMENTS (IDs) with
signature of depositor (e.g. Driver’s License, SSS/ GSIS
ID, Senior Citizen’s ID, Passport, PRC ID, OWWA/ OFW ID,
Seaman’s ID, Alien Certification of Registration ID, Voter’s
ID);
ORIGINAL COPY AND PHOTOCOPY OF THE
BIRTH CERTIFICATE FROM THE NATIONAL STATISTICS OFFICE (NSO) OR
DULY CERTIFIED COPY FROM THE LOCAL CIVIL REGISTRAR, for
depositors below eighteen (18) years old.
Other documents may be required by the PDIC
representative/s in the course of their processing of claims
filed.
All claims shall be processed in the PDIC
Home Office. Check payments for valid deposit insurance claims
will be sent through registered mail.
In accordance with the provisions of R.A.
3591, as amended, the last day for filing claims for insured
deposit in the closed RB Ayungon, RB Lagonoy and RB
Hinigaran is on October 15, 2012. After
October 15, 2012, PDIC, as insurer, shall no longer accept any
claim for insured deposits maintained with the said closed
bank.
