Notice to Depositors of Closed Legacy-Affiliated Banks

  1. The onsite distribution of claim forms and receiving of deposit insurance claims at the following Legacy affiliated banks ended on April 3, 2009:


    1. Bank of East Asia (Bangko Rural de Bisayas [Minglanilla, Cebu]), Inc.
    2. Pilipino Rural Bank, Inc.
    3. Rural Bank of Carmen (Cebu), Inc.
    4. Philippine Countryside Rural Bank
    5. Rural Bank of DARBCI, Inc.
    6. San Pablo City Development Ban
    7. First Interstate Bank (Rural Bank of Kananga, Leyte), Inc.
    8. Dynamic Bank
    9. Rural Bank of San Jose (Batangas), Inc.
    10. Nation Bank
    11. Rural Bank of Bais (Negros Oriental), Inc.

  2. In the interest of public service, the Philippine Deposit Insurance Corporation (PDIC) has extended the schedule of distribution of claim forms and receiving of deposit insurance claims.

  3. For expediency, the PDIC has set up four (4) Regional Claims Receiving Centers in Metro Manila, Luzon, Visayas and Mindanao to attend to depositors of the abovementioned banks.

  4. PDIC representatives will be at the designated Regional Claims Receiving Center on the periods specified below.

  5. Depositors may transact at the Regional Claims Receiving Center nearest their place of residence to get claim forms and/or to file their claims.

  6. Depositors with deposit accounts in two (2) or more Legacy-affiliated banks may file their claims in just one (1) Regional Claims Receiving Center, nearest their place of residence.

  1. GENERAL GUIDELINES FOR GETTING CLAIM FORMS

    1. Present original evidence of deposit such as savings passbook, certificate of time deposit, bank statements including unused checks, to the PDIC representative designated in the Regional Claims Receiving Center/RB Paranaque servicing sites.

    2. Distribution of claim forms is on a “first come, first serve” basis.

  2. GENERAL GUIDELINES FOR FILING CLAIMS FOR DEPOSIT INSURANCE:

    1. PERSONAL APPEARANCE shall be required of ALL depositors, except when depositor is:

      1. below seven (7) years old;

      2. abroad; or

      3. physically incapable of personally appearing before the PDIC Claims Personnel due to serious physical injury, condition, ailment, or infirmity due to advanced age.

      NOTE: Personal appearance shall be required of depositors aged 7 to 17 years old.

    2. The following REQUIREMENTS are needed when filing claims:

      1. DULY ACCOMPLISHED CLAIM FORM (complete entries on the white, green & yellow copies);

      2. ORIGINAL EVIDENCE OF DEPOSIT SUCH AS SAVINGS PASSBOOK, CERTIFICATE OF TIME DEPOSIT, OR BANK STATEMENT INCLUDING UNUSED CHECKS;

        For joint accounts: “OR, AND/OR, AND”, evidence of deposit should be photocopied as many copies as there are depositors to the account.

      3. ORIGINAL AND PHOTOCOPY OF TWO (2) VALID photo-bearing IDENTIFICATION DOCUMENTS (IDs) with signature (e.g. Driver’s license, SSS/GSIS ID, Senior Citizen’s ID, Passport, PRC ID, OWWA/OFW ID, Seaman’s ID, Alien Certification of Registration ID, Voter’s ID, Company/ School ID, Postal ID)

      4. COPY OF THE BIRTH CERTIFICATE DULY CERTIFIED BY THE NATIONAL STATISTICS OFFICE OR THE LOCAL CIVIL REGISTRAR, for depositors below eighteen (18) years old.

      5. For depositors below eighteen (18) years old, parent must file claim.

  3. All claims will be processed in the PDIC Head Office. Check payments for valid deposit insurance claims will be sent through registered mail.

    IMPORTANT REMINDERS:

    1. There are NO fees or charges for the processing or payment of deposit insurance claims.

    2. PDIC Claim Forms, pro forma affidavits and documents are given free of charge.

    3. The public is advised to transact only with authorized PDIC representatives stationed at the Regional Claims Receiving Centers/RB Paranaque servicing sites. Do not transact with brokers or fixers.
